Booking an Initial Visit

To form a relationship between a physician and a patient we require that all patients have an inital visit with their new doctor.

The purpose of this visit is to give the doctor time to review your health history as well as review any policies, procedures or expectations that either the patient or doctor may have of each other.

**Note: Due to the shortage of family doctors we ask that if you already have a doctor that you continue to work with your existing doctor through medical issues or care issues you may have. 

The clinic also has a policy against changing doctors internally without informing your previous physician that you no longer need their care.

For Families, Please book each member of the family individually

BOOKING INSTRUCTIONS:

1) Click the Link Above or click HERE

2) Enter your Name, 9 Digit health number (PHIN) and your Birthday then click “Check-in”

If you already have a file in our system then it will proceed to the next step. 
If you do not have a file in our system it will ask you to confirm your details

3) Choose a Doctor accepting new patients

4) Choose an appointment type

We are only offering initial appointments at this time

5) Pick your date & time

That is all.  We will see you there.
